Terms of Agreement

• I agree for my child to attend sprint tutorials.

• I agree to pay the payment of £10 registration fee.

• I agree to the payment of fees on the 1st of the month in advance.

• I understand that the monthly fee is for four weeks. I also understand that the number of weeks per calendar month varies and when these vary for the month, the fees will be adjusted accordingly.

• I agree to provide Sprint Tutorials with at least one full calendar month’s notice in writing to terminate tuition and I understand that tuition will then end at the end of the following calendar month (e.g. to terminate tuition on 30/11 notice is required on 30/10).

• I understand that fees are still payable for sessions my child does not attend.

• I give permission for a trained member of staff to administer appropriate first aid to my child if required.

• I confirm that the information given on this form is correct and agree to notify Sprint Tutorials of any changes.

• I understand that it is my responsibility to ensure my child arrives at their study session on time, and is either collected at the end of their session by one of the named adults on this form, or is permitted to make their own way home.

• I agree that Sprint Tutorials may contact me with regards to my child or their services using any of the contact details I have provided.

• I understand that Sprint Tutorials may change the cost of sessions by providing at least two calendar months notice to me, or instantly in case that an applied discount is no longer applicable.

• I understand that Sprint Tutorials may cancel this agreement at any time.
